Nuchal Translucency Quality Assurance (NTQA) Registration
Quality assurance is integral to any screening process. Prenatal Screening Ontario is mandated to ensure that prenatal screening in Ontario is held to high quality standards. Please see the options below to register for a BORN login under your existing NT ID number or to register for a new NT ID number.
For further important information on NTQA in Ontario please see the Prenatal Screening Ontario website NTQA information.
Note: All sonographers and physicians who provide NT measurements for prenatal screening are being asked to register with BORN and Prenatal Screening Ontario, and to access their NTQA audit curve. Registration will require providing up to date contact information so that PSO can provide important information regarding NTQA in the province (please fill out the “existing sonographer” form below to register for a BORN login ID). All future communications regarding NTQA will occur via the email in your BORN profile. If you are already registered with BORN, please update your contact information no later than April 1, 2019. Failure to provide updated contact information may result in missing communications pertaining to mandatory NTQA requirements, and can result in suspension or loss of NT ID number.
To update your contact information please follow these steps:
1. Login to the BORN Information System with your username and ID
2. Click on “My Profile”
3. Click on "Edit my Profile"
4. Input your email address in the Email Address box
5. Click on "save"
Select one of the three roles below to launch the registration form. Your contact information will not be shared without your consent. Once you have submitted your contact information, you will receive an email with further information. If you do not receive this email confirmation, please contact us.
I certify that the information provided in this application is true and correct and I am making this application on my own behalf. J’atteste que les renseignements fournis dans ce formulaire d’inscription sont exacts et que je présente cette demande pour moi‑même.
*Note to ALL registrants:
If you are using a CHROME browser and receive an error message when entering either your FMF number or your Prenatal Screening ID number, please try again in a different browser (i.e. Internet Explorer or Mozilla Firefox).
*Note to EXISTING registrants:
Your Prenatal Screening ID number is the sonographer code you put on the first trimester screening blood work requisition form when you complete an NT examination. For most sonographers this is the same as their FMF number. However, for some sonographers their FMF code and their Prenatal Screening ID number are different depending on when they registered.
